TERMS OF USE FOR TRAVEL WELL CARE LLC

Welcome to the website of Travel Well Care LLC. These Terms of Use (“Terms”) govern your use of our website and services. By accessing our site or booking an appointment, you agree to be bound by these Terms.

1. MEDICAL EMERGENCY DISCLAIMER

IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 9-1-1 IMMEDIATELY OR GO TO THE NEAREST EMERGENCY ROOM. Travel Well Care LLC provides non-emergent, acute care consultations. We are not an emergency service provider.

2. LICENSURE AND SCOPE OF PRACTICE

Travel Well Care LLC is a Rhode Island-based professional entity located at 6 Alcar Drive, Johnston, RI 02919.

Provider: Lauren Silva, MSN, APRN, FNP-C.
Licensure: Licensed in the State of Rhode Island.
Florida Practice: Registered as an Out-of-State Telehealth Provider with the Florida Department of Health.
Service Area: We only provide clinical services to individuals physically located in Rhode Island or Florida at the time of the consultation.

3. NO PROVIDER-PATIENT RELATIONSHIP

Browsing this website, viewing social media content, or communicating via email (travelwellcare@gmail.com) does not establish a provider-patient relationship. A formal relationship is only established once you have:

Completed the intake and registration forms.
Agreed to the Informed Consent.
Commenced a formal clinical consultation with Lauren Silva, NP.

4. STANDARD OF CARE & TELEHEALTH LIMITATIONS

We aim to provide care that meets the same professional standard as in-person services. However, telehealth has inherent limitations (e.g., the inability to perform a physical exam).

Referrals: If the provider determines that your condition requires an in-person exam, diagnostic testing, or emergency intervention, we reserve the right to terminate the session and refer you to a local facility.

5. PRESCRIPTION POLICY

No Controlled Substances: We do not prescribe controlled substances (Schedule II-V) via telehealth under any circumstances.
Clinical Discretion: Prescriptions are issued only when clinically indicated. A consultation does not guarantee a prescription.
Pharmacy: Prescriptions will only be sent to pharmacies in states where the provider is legally registered to practice.

6. CONSENT FOR TREATMENT OF MINORS

Travel Well Care LLC specializes in family travel.

Presence Required: A parent or legal guardian must be present on camera during the entire consultation for any patient under the age of 18.
Authorization: By booking for a minor, you represent that you have the legal authority to consent to their medical treatment.

7. PATIENT CONDUCT & PRIVACY ENVIRONMENT

To comply with HIPAA and safety standards, patients must:

Be Stationary: Consultations will not be conducted while the patient is operating a motor vehicle.
Be in Private: You must be in a private, quiet indoor location to ensure confidentiality.
Be Prepared: Ensure you have a stable internet connection. Travel Well Care LLC is not responsible for connectivity issues on the patient's end.

8. PAYMENT, CANCELLATION, AND REFUNDS

Payment: Payment is due at the time of booking or prior to the start of the session.
Cancellations: Appointments canceled with less than 24 hours' notice are subject to a $50.00 late fee.
No-Shows: If you are more than 10 minutes late to your digital session, it will be considered a "No-Show" and the full fee will be charged.
Refunds: We do not offer refunds for completed consultations, regardless of the clinical outcome or whether a specific medication was prescribed.

9. MODIFIED AVAILABILITY (JUNE – AUGUST 2026)

Please be advised that Travel Well Care LLC will operate on a limited/modified schedule from June 2026 through August 2026 for maternity leave. During this time, same-day or urgent "on-call" services may be unavailable. Please refer to our live booking calendar for current availability.

10. GOVERNING LAW

These Terms are governed by and construed in accordance with the laws of the State of Rhode Island, without regard to conflict of law principles. Any legal action shall be brought exclusively in the courts located in Providence County, RI.
